Diocesan Release Forms
The required release forms for any adult and/or minor who wishes to volunteer and/or participate in a diocesan event in the Diocese of Fort Worth.
Online Forms
These youth ministry release forms will take effect June 1, 2026 and remain in effect until June 30, 2027.
Form A — Annual Youth Ministry Parental Liability Waiver, Permission and Medical Information
This form must be completed by the parent for every one of their children attending any parish / diocesan event annually beginning anytime after May 31st. Each child must have their own separate form.
Form B — Consent to Participate, Code of Conduct, and Consent for Emergency Medical Treatment
This form must be complete by the parent for every event in which their child(ren) participates. For example, a child participating in VBS, Faith Formation, and a Youth Ministry Conference will have 3 separate Form B's during the catechetical year.
Form D — Adult Liability Waiver, Medical Release and Promotional Release Form
For all adults participating in parish / diocesan youth ministry.
Note: Must be at least 18 years old or older and have graduated from High School.
Out of State Form A — Annual Youth Ministry Parental Liability Waiver, Permission and Medical Information
Per each child attending any parish / diocesan youth event annually beginning anytime after May 31st.

Overnight Retreat Request Form
Please use the form below to request permission to host and or direct an overnight retreat within the Diocese of Fort Worth. The present form must be turned in at minimum (2) months prior to the scheduled event.
Por favor use esta forma para solicitar permiso para un retiro nocturno o que se llevara a cabo durante la noche en la Diócesis de Fort Worth. Esta forma debe entregarse con un mínimo de 2 meses de anticipación al evento.
