Vocations Office Diocesan Forms

The required release forms for any adult, volunteer, or child who wishes to participate in a diocesan vocation event in the Diocese of Fort Worth.

Release Forms 2024 - 2025

Vocation Events Only

These vocations office release forms will take effect June 1, 2024 and remain in effect until May 31, 2025.

Form A — Annual Vocations Office Parental Liability Waiver, Permission and Medical Information

Per each child attending any diocesan vocation office event annually beginning anytime after May 31st.

Form B — Consent to Participate, Code of Conduct, and Consent for Emergency Medical Treatment

Must be filled out per each diocesan vocation office event that the teenager is attending. This form “Consent to Participate, Code of Conduct, and Consent for Emergency Medical Treatment” must be attached to the Parent/Guardian/Conservator Permission, Liability Waiver, and Medical Information (FORM A) for each event attended. Forms A and B must travel to and from each trip.

Form D — Adult Liability Waiver, Medical Release and Promotional Release Form

For all adults participating in diocesan vocation office event.
Note: Must be at least 18 years old or older.

Out of State Form A — Annual Vocations Office Parental Liability Waiver, Permission and Medical Information

Per each child attending any diocesan vocation office event annually beginning anytime after May 31st. Forms OA (Out of state) and B are required for all Out of State events.
